Job Advert: Reablement Support Worker
Pay: £13.47 per hour
Contract: Initial 3-month temporary (with potential temp-to-perm)
Location: Service users’ homes across Swindon and surrounding areas
Hours: Flexible shifts – days, evenings, weekends and bank holidays (5 days over 7, within agreed availability; short-notice cover may be required)
About the role
Join the Reablement Team as a Support Worker and help vulnerable adults regain, improve and maintain the skills they need for independent living in their own homes. You will deliver person-centred personal care and reablement support, always treating people with dignity and respect. The role also includes supporting people at the end of life and may involve working with other service-user groups as directed.
You will work as part of a multidisciplinary Adult Health & Social Care team, encouraging independence, monitoring progress against support plans, and liaising with professionals, families and carers.
Key responsibilities
- Provide prompting and hands-on support with personal care and daily living skills
- Continuously assess needs and feed back so support can be reduced or stopped when appropriate
- Support with medication administration (in line with policy), therapy regimes, kitchen skills and use of Telecare/Telehealth equipment
- Help with practical tasks such as shopping, bill paying or pension collection where required
- Maintain accurate records of visits, medication, incidents and near misses
- Provide physical and emotional support for people nearing end of life
- Work flexibly across shifts (including evenings, weekends and bank holidays) and respond to emergency situations
- Attend training, supervision, team meetings and support the induction of new colleagues
What we’re looking for
Essential
- NVQ Level 2 in Care (or willingness to work towards it)
- Awareness of issues affecting vulnerable people (including discrimination) and knowledge of person-centred care
- Strong communication skills and ability to work confidentially with service users, families and professionals
- Literate and numerate
- Physically fit and flexible – able to work in confined spaces, assist with transfers, bending, etc.
- Able to travel around Swindon and outlying areas to meet time-sensitive appointments
- Commitment to safeguarding, dignity, respect and the Council’s values (We See, We Hear, We Care, We Act, We Learn)
Desirable
- Awareness of health & safety, moving & assisting
- Ability to monitor progress and adapt approach flexibly
