As our Occupational Therapist, you will assess residents' physical, cognitive, communication, and functional abilities, developing personalised interventions that support independence and quality of life. You will work closely with nurses, carers, families, GPs, and other healthcare professionals to ensure residents achieve their individual goals and maintain the highest possible level of function.
Responsibilities
- Assess residents' physical, communication, interaction, and cognitive skills.
- Develop and implement individual treatment programmes and meaningful activities.
- Contribute to short-term and long-term care goals as part of the multidisciplinary team.
- Recommend and organise adaptations to residents' rooms and living environments.
- Assess, recommend, and support the use of specialist equipment.
- Provide advice and exercises to improve mobility, dexterity, and independence.
- Offer education and support to residents, relatives, and carers.
- Maintain accurate clinical records and case notes.
- Liaise effectively with healthcare professionals, families, and external agencies.
- Monitor progress and review treatment outcomes.
- Participate in multidisciplinary meetings and case conferences.
- Support residents in finding new ways to approach everyday tasks and activities.
You will:
- Be a qualified Occupational Therapist with current HCPC registration.
- Have excellent assessment and communication skills.
- Be passionate about person-centred care and promoting independence.
- Enjoy working collaboratively within a multidisciplinary team.
- Be organised, compassionate, and proactive.
- Have experience working with older adults, rehabilitation, or care home environments (desirable but not essential).
