Working with a supportive rehabilitation team, you’ll deliver hands‑on care so patients can regain independence and quality of life.
You’ll be part of a caring, multi‑disciplinary team within an Intermediate Care Service, supporting people through their recovery. Your work will help create a safe, respectful environment where each person’s needs, goals and preferences are at the centre of care.

What you’ll do:
Support patients with day-to-day care based on their individual care plans
Carry out basic clinical tasks such as observations, blood pressure and sample collection
Communicate clearly with patients, carers and colleagues
Help prepare clinical areas and maintain equipment and stock levels
Record accurate patient information using electronic systems
What we’re looking for:
A Level 2 qualification in Health and Social Care, or similar experience
Experience supporting patients in a healthcare, home care or care home setting
Kind, clear communication skills, including with people who may feel distressed
Ability to work well in a team and manage your own tasks
Willingness to complete the Care Certificate if not already held
