Would you like to work in the community as part of our team of Occupational Therapists and Support Workers, creating person-centred personal plans helping people set and achieve their goals following an illness? Then this role is for you.
We have opportunities available to visit people in their own homes in Norwich and surrounding areas. Norfolk First Support delivers services to adults across Norfolk in their own homes for up to 6 weeks, assisting them to regain skills they may have lost following an illness or hospital admission, enabling them to regain their independence as much as possible while also reviewing any ongoing support needed.
In this role, you will be part of a multidisciplinary assessment team, working alongside other professionals in Social Care supporting people who may have been discharged from hospital or where they have a change of circumstances and require support at home.
You will assess and review their daily living needs, developing a personalised plan to assist them to regain the skills and confidence to remain independent in their home and, if ongoing services are required, complete a Care Act Assessment. All training will be provided to ensure you are competent to deliver this critical role.
You will need to have experience of working with people and be able to communicate clearly and appropriately with members of the public, whilst exercising judgement and using your initiative to plan and prioritise your work. You will also need knowledge of current policy, practice and legislation within Health and Social Care.
The post is currently Monday to Friday. Weekend working may be reintroduced in the future on a rota basis or as needed by the service.
