Due to operational requirements and continued service demands, we are recruiting a Care Assessor to join our team.
We are looking for an experienced professional to join our busy team.
This is a varied role combining community-based assessments with office-based care planning and operational support. You will complete new service user assessments, develop person-centred care plans and risk assessments, review existing care documentation and work closely with service users, families and healthcare professionals to ensure the highest standards of care.
This is not a home-working role. When not undertaking community visits, you will be based within our Norwich office, working alongside our care management team and supporting the day-to-day operation of the service.
Key Responsibilities
- Complete new service user assessments.
- Develop and review person-centred care plans and risk assessments.
- Support the onboarding of new service users.
- Maintain accurate records using Access Care Planning.
- Build positive relationships with service users, families and professionals.
- Provide office-based support, including answering telephone enquiries.
- Participate in the on-call rota.
What We're Looking For
- Experience within domiciliary care.
- Excellent knowledge of care planning and risk assessment.
- Professional, well-mannered and confident communicator.
- Highly organised with excellent attention to detail.
- Able to manage a busy and varied workload.
- Proficient in IT (experience of Access Care Planning or similar is desirable).
- Full UK driving licence and access to your own vehicle.
