Job Overview
We are seeking a dynamic and proactive Primary Care Network (PCN) Manager to lead and coordinate integrated healthcare services within a vibrant community setting. Duties
- Lead the strategic planning and operational management of the PCN to ensure efficient service delivery.
- Coordinate with general practitioners, specialists, allied health professionals, and community organisations to promote integrated care pathways.
- Oversee the development of policies, protocols, and workflows that support high-quality patient care and team collaboration.
- Monitor performance metrics, analyse data, and implement continuous improvement initiatives to enhance service effectiveness.
- Facilitate communication among team members, ensuring clarity of roles and responsibilities while fostering a culture of teamwork and accountability.
- Manage budgets, resources, and staffing levels to meet organisational goals while maintaining compliance with healthcare regulations.
- Support staff development through training programs, mentorship, and performance evaluations to cultivate a motivated and skilled team.
Skills
- Proven management experience in healthcare settings or community health programs.
- Exceptional leadership abilities with a talent for motivating multidisciplinary teams.
- Strong organisational skills with an eye for detail in planning and executing complex projects.
- Excellent communication skills to engage effectively with healthcare professionals, patients, and external partners.
- Strong knowledge and experience of HR policies, employment legislation and people management, including recruitment, employee relations, performance management, workforce planning and staff development.
- Analytical mindset capable of interpreting data to inform decision-making and improve service quality.
- Knowledge of healthcare policies, regulations, and best practices related to primary care services.
- Ability to adapt quickly to changing priorities while maintaining focus on strategic objectives.

- General Management: 10 years (required)
