Care Coordinator – Winchester City Primary Care Network (PCN)

Due to continued success and expansion, Winchester City Primary Care Network (PCN) is looking for an enthusiastic and motivated Care Coordinator to join our friendly, supportive, and forward-thinking team.
Working across our three GP practices in central Winchester, you’ll play a key role in helping patients access the right care at the right time—while supporting clinicians to focus on delivering high-quality, patient-centred care.
You’ll work alongside our experienced Care Coordinators and closely with the PCN Leadership Team, reporting to the Operations Manager to help ensure effective workload management, smooth coordination of services, and excellent patient outcomes.
What you’ll do
- Work collaboratively with GPs, nurses, and wider healthcare professionals to coordinate patient care pathways.
- Liaise with hospitals, community services, and external agencies to ensure seamless communication and continuity of care.
- Provide vital administrative and organisational support, including processing referrals, managing clinical systems, and supporting meetings.
- Support projects and initiatives aimed at improving population health across Winchester.
- Maintain accurate, timely records and uphold strict confidentiality in line with data protection and governance standards.
- Collaborate with fellow Care Coordinators across the PCN, sharing best practice and providing cross-cover when needed.
- Work closely with the PCN Leadership Team, handling sensitive information with professionalism and discretion.
About you
We’re looking for someone who is:
- Highly organised, proactive, and passionate about making a difference to patient care.
- A confident communicator who enjoys teamwork, problem-solving, and building strong working relationships.
- Comfortable using Microsoft Office and clinical systems (experience with EMIS is advantageous, but training will be provided).
- Adaptable and keen to learn in a dynamic and evolving primary care environment.
- Trustworthy and professional, with the ability to manage confidential and sensitive information appropriately.
Experience in primary care or a medical secretary/administrative role is desirable but not essential—we are keen to support the right candidate with the skills, attitude, and potential to succeed.

Pay: £13.50-£14.56 per hour
