In this role, you will play a key part in supporting individuals across the Colwyn Bay area who use Stori services, helping them to make their own choices, build confidence, develop practical skills, and live as independently as possible. You will encourage and motivate people to take an active role in shaping their lives and achieving their personal goals.
Adopting a compassionate, respectful and person-centred approach, you will work alongside individuals to identify their needs and connect them with appropriate external services, including healthcare, financial and legal support. Your role will involve building positive relationships, working collaboratively with professionals, and promoting wellbeing, independence and empowerment at every stage.
Candidates must hold a full driving licence, have access to a vehicle and be able to prove their right to work in the UK (we do not offer sponsorship visas).
This role is subject to an Enhanced DBS disclosure check (Adult & Child Workforce), which will be funded by the company if required.
