Band 6 Children’s Community Nurse – CCN & SNSN
We are seeking a experienced and enthusiastic Children’s Nurses to join our Community Children’s Nursing Services. This Band 6 role combines working within the Special School Nursing Service (SNSN) during term time and the Children’s Community Nursing (CCN) Team during school holidays, supporting CYP with complex health needs across educational and community settings.
During term time, you will provide specialist nursing care within special schools, supporting CYP with complex medical conditions, disabilities, and long-term health needs. Responsibilities include care planning, health assessments, medication management, enteral feeding, epilepsy management, staff training, and contributing to Education, Health and Care Plans (EHCPs).
During school holidays, you will work within the CCN Team, delivering nursing care in homes and the community. You will support CYP with complex and technology-dependent needs, including tracheostomy care, continuing care, palliative care, and hospital discharge support.
As a Band 6 nurse, you will provide clinical leadership, supervise and support colleagues, contribute to service development, and work collaboratively with families, schools, therapists, social care, and medical teams to deliver high-quality, child-centred care.
This role is ideal for a dedicated nurse with experience in caring for children with complex health needs, community nursing or special school nursing.
You will work autonomously managing a caseload of children and young people assessing, planning, implementing and evaluating their nursing care. Your activities will be varied, including, but not limited to delivering clinical interventions, training parents, carers and staff from other agencies, and working with multi professional and multi-agency colleagues to support our children to achieve their best outcomes.
Our wider service is extensive and varied and can offer opportunities to work with children who are technology dependant, children’s palliative care, respite care for medically complex children, children’s continuing care and medically complex children in our special school nursing. No two days are the same, but each day counts.
You will work in collaboration with your colleagues to deliver end of life care to children at home, when this is their chosen place of death. To do this you will be part of our 24 hour on call rota and will be supported by a compassionate and caring team who care for each other as we care for our patients.
