Job Summary
Empower service users with tailored support, guidance and advice to help them make healthy lifestyle changes, improve their wellbeing and work towards better long-term health.
This is an all-round community-facing role, requiring the postholder to work actively within local communities, Saheli Hub sites, wellbeing centres, GP practices and partner organisations. The role will involve outreach, building trust with residents and local partners, encouraging referrals, and supporting people to access the right services at the right time.
Support service users to understand how their diet can impact their long-term health, including helping them make informed food choices and avoid foods that may increase their sugar levels or affect their overall health.
Help service users build their knowledge and confidence around what they eat, how food affects their body, and how small everyday changes can support healthier living.
Support service users to make positive changes in their daily lives by increasing social and physical activity in a way that is suitable for their age, ability and personal circumstances. This should help improve their chances of better long-term health and reduce the need for future medical or clinical intervention where possible.
Provide opportunities for service users to build friendships, feel a sense of belonging, and develop their knowledge, skills, confidence and enjoyment of physical activity, wellbeing activities and community-based support.
Ensure each service user is signed up to the CRM and has completed a lifestyle assessment. Maintain a clear overview of each service user’s journey, including their attendance, progress, improvements in lifestyle indicators, health and wellbeing outcomes, and any relevant case studies or stories of difference.
Provide a holistic and person-centred service by co-designing personalised support plans, including GP or social prescribing pathways where appropriate. Meet with service users, complete triage, provide support, and signpost them to services that can improve their diet, health and wellbeing.
Promote and encourage referral uptake from within Saheli sites, wellbeing centres, GP practices, local community groups and partner organisations. Support service users to access the full range of Saheli Hub services to help reduce the need for clinical or medical interventions where possible.
Where appropriate, refer service users to health professionals, support providers or other agencies, and review the outcome to make sure their needs are being met. Ensure service users are supported throughout the intervention and that safeguarding procedures for vulnerable people are always followed.
Manage and monitor your caseload in line with the priorities set by the Saheli Hub Manager. Regularly review and assess the effectiveness of the service, identifying areas for improvement and sharing ideas with your manager to strengthen delivery and outcomes.
Pay: £26,000.00-£28,000.00 per year
Ability to commute/relocate:
- Birmingham B8 3LT: reliably commute or plan to relocate before starting work (required)
Education:
- GCSE or equivalent (preferred)
