Due to continued growth, we are looking to recruit an Junior Placement Manager to join our team in South west.
The Role
The Placement Manager will play a key role in the coordination and management of care placements, ensuring that individuals receive safe, effective and person-centred support. The successful candidate will oversee the delivery of care packages, supervise staff, and ensure that all care planning and risk management processes are completed to a high standard.
This role requires strong leadership skills, the ability to work collaboratively with professionals and families, and a commitment to achieving positive outcomes for the individuals we support.
Key Responsibilities
Managing and overseeing placements to ensure that care is delivered safely and in line with agreed care plans and risk assessments.
Leading on the development, implementation and review of person-centred care plans and risk assessments.
Completing initial assessments for new referrals and supporting the transition into services.
Supervising and supporting care staff to ensure high standards of practice and compliance with regulatory requirements.
Supporting safeguarding processes and ensuring that any concerns are appropriately escalated and managed.
Monitoring care delivery to ensure that services meet CQC standards and organisational policies.
Working closely with families, professionals and service users to ensure that support plans reflect individual needs, goals and outcomes.
Essential Requirements
NVQ Level 3 in Health and Social Care (or above).
Experience supervising staff within a health or social care setting.
Experience developing care plans and completing risk assessments.
Experience completing assessments for new service users.
Experience working with social workers and attending professional meetings.
Knowledge and understanding of safeguarding procedures.
Strong organisational, communication and leadership skills.
A commitment to person-centred care and achieving positive outcomes.
Desirable
NVQ Level 4 or Level 5 in Health and Social Care or Leadership and Management.
Experience working with individuals with complex mental health needs, learning disabilities or autism.
Experience within supported living, community care or reablement services.
