We are looking for a compassionate and motivated Wellbeing Link Worker to join our Primary Care Network team in Farnborough.
This role is nationally known as a Social Prescribing Link Worker. You will support people whose health and wellbeing are affected by social, emotional or practical issues, helping them identify what matters to them and connect with appropriate local services, community groups and activities.
About the role
You will:
- Meet with patients and hold personalised “what matters to you” conversations
- Help people identify their goals and create a wellbeing plan
- Support individuals experiencing loneliness, financial difficulties, housing concerns, low confidence, caring responsibilities or other social challenges
- Connect people with community groups, voluntary organisations and statutory services
- Work closely with GP practices, health professionals and local partners
- Manage a caseload and maintain accurate, confidential records
- Identify safeguarding concerns and escalate appropriately
- Help improve access to services and reduce health inequalities
This is a non-clinical role and does not involve diagnosis, treatment, counselling or crisis intervention.
About you
You will have:
- Experience of supporting people in health, social care, community, voluntary or advice services
- Excellent listening, communication and relationship-building skills
- A compassionate, non-judgemental and person-centred approach
- The ability to work independently and as part of a multidisciplinary team
- Strong organisation and record-keeping skills
- An understanding of confidentiality, safeguarding and professional boundaries
- Knowledge of Farnborough and local community services would be an advantage
You will be required to travel between GP practices and community locations across Farnborough. An appropriate DBS check will be required.
