Central London Community Healthcare (CLCH) is seeking a motivated and innovative Band 7 Senior Nurse to join our developing Integrated Care Service . This is an exciting opportunity to work alongside NHS acute, community and primary care providers, Local Authorities and Third Sector organisations to deliver truly integrated, person-centred care.

You'll work closely with patients, families and carers to ensure individuals receive coordinated care, personalised treatment plans and timely access to the services they need.

The Case Manager will be a key member of the Integrated Care Team (ICT), contributing to the delivery of holistic, integrated care for patients in West London CCG. The role will involve a blend of care coordination and direct care delivery, working to ensure the most effective care for patients with complex needs.
Key responsibilities include:
Managing a caseload of complex patients, empowering them and their carers to self-care and self-manage where possible.
Collaborating with patients, their families and the multidisciplinary team to develop personalised care plans.
Coordinating care and services across health and social care sectors to improve outcomes and prevent duplication.
Supporting prevention initiatives and promoting self-care to reduce the risk of avoidable hospital admissions.
Working closely with colleagues in MDT, GPs, District Nurses, Social Workers and Pharmacists to assess and address patient needs.
Contributing to regular MDT meetings and service development initiatives, sharing insights and feedback.
Providing operational support to Health and Social Care Assistants as needed.
Delivering high quality care by aligning services with key performance indicators and promoting holistic care.
Initiating appropriate clinical interventions to improve patient outcomes and quality of life.
