Job Summary
We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and managing care services, ensuring clients receive personalised support tailored to their individual needs. The Care Coordinator will work closely with clients, families, and healthcare professionals to facilitate seamless care delivery.
Duties
- Coordinate and schedule care visits efficiently to meet client needs.
- Manage staff rotas and ensure adequate cover for all care calls.
- Organise and schedule care plans in collaboration with clients, families, and healthcare providers
- Act as the primary point of contact for clients, providing ongoing support and guidance
- Maintain accurate and up-to-date records of client information and care activities using IT systems
- Coordinate with care staff to ensure timely delivery of services and address any scheduling conflicts or issues
- Monitor client progress and adjust care plans as necessary in consultation with relevant professionals
- Provide clear communication regarding care procedures, schedules, and updates to all stakeholders
- Assist in the onboarding of new clients by explaining services, answering questions, and completing necessary documentation
- Support training sessions for staff or clients as needed, utilising teaching skills to enhance understanding
Essential Requirements
- Minimum NVQ Level 3 in Health and Social Care (or equivalent).
- Previous experience as a Care Coordinator within a domiciliary care setting.
- Good understanding of CQC regulations and care standards.
- Excellent communication and organisational skills.
- Strong IT skills and experience using care management systems.
- Ability to manage multiple priorities in a fast-paced environment.
- Full UK driving licence and access to a vehicle.
- Right to work in the UK.
Desirable Requirements
- NVQ Level 5 in Health and Social Care or willingness to work towards it.
- Experience with care planning, staff and rota management software.
- Experience supervising or mentoring care staff.
