About the Role
We are looking for passionate and committed individuals to join our Home First Service as Community Support Workers (CSWs). We currently have vacancies in our Hospital Discharge team, Central Duty Team and Early Review.
Our Hospital Discharge Team are based at the Leicestershire Royal Infirmary and work across the three acute Leicester hospitals. This is a dynamic team who work with Health colleagues, as part of a multi-disciplinary team to ensure patients leave hospital timely and are well supported. Use of the Hospital Hopper bus service means this position is open to people who cannot drive or do not have access to a motor vehicle.
Our Central Duty Team are a countywide team based at County Hall. The team work with people of Leicestershire who are new to services and or require urgent care and support. Again, this is a dynamic position that requires you to think on your feet, working at pace to problem solves to ensure people are well support and able to remain at home were possible. The team work across the County of Leicestershire; a car is essential to this post.
The Early Review Team are also a countywide team who work alongside the hospital and central duty team. The team visit people who have recently been discharged from hospital and or who are new to services to ensure they have the right level of care in place. The team is critical to ensuring people remain as independent as possible. The team work across the County of Leicestershire; a car is essential to this post.
This career-graded role offers a structured progression route from Grade 8 to Grade 9, supporting adults, their families and carers in line with the Care Act 2014 and Mental Capacity Act 2005.
At Grade 8, you will begin your journey with a comprehensive Learning and Induction Programme, including the Care Certificate and Diploma in Care Level 2. With mentoring from Senior CSWs, you’ll develop the skills and experience needed to progress to Grade 9, where you’ll take on more complex responsibilities including assessment, support planning, and case management.

- Brokerage Team (Permanent, 37 Hours Per Week)
Discharge Hub (Permanent, 37 Hours Per Week)

About You
To apply for the CSW post, you must:
- Level 2 qualification (or equivalent) in Maths and English and a relevant health/social care qualification or equivalent experience
- Understanding of adult social care needs and risk assessments
- Ability to undertake direct work with those who access or need the service
- Strong communication, teamwork, and partnership working skills
- Demonstrated ability to manage caseloads and deliver person-centred care
- Good written and verbal communication and negotiation skills
The ability and means to travel across the county of Leicestershire to undertake visits

This role is part of a structured career pathway. Upon successful completion of training and probation, you will progress to Grade 9. Opportunities to apply for Grade 10 (Senior CSW) roles are available via competitive interview, offering further development and leadership responsibilities.
