An exciting opportunity has arisen for a part time enthusiastic, vibrant, Physical Health & Wellbeing Nurse, to work in our community branch of the Mental Health Rehabilitation Service, we are based at the St Austell Hospital site.
The Community Mental Health Rehabilitation Team provides a service across the whole county of Cornwall supporting people recovering from complex and enduring mental health issues. The Community Rehabilitation Team operates alongside the inpatient facility Fettle House allowing the team to follow through a service offering a specific pathway for the patients as they resume/establish independent lives outside of the hospital setting.
The Community Rehabilitation Mental Health Team works collaboratively as part of a wider structure within Cornwall Foundation Trust, this includes carers, families, other specialists e.g., Early Intervention Team, we also assist with the discharges of patients from the acute wards and work alongside housing providers, education, pharmacy.
The post holder will be a key driver in overseeing the physical health of individuals on caseload, this will include completing annual physical health checks, liaising with GP’s for any essential health monitoring and follow ups.
We take a person-centred approach in developing individualised dialogue plus care plans. In addition, the post holder will have a key role supporting and supervising junior staff, as well as participating in the continued review and development of the rehabilitation service. Regular individual support and supervision is provided. This is a part-time post working 09:00 – 17:00 days negotiable.

A post for a dynamic passionate nurse that wants to use all their general holistic nursing skills to help provide the best nursing care to our adult mental health patients in Cornwall.
The post holder will be a visible, accessible and professional authoritative presence for patients and staff, working alongside staff in teams in direct clinical contact with patients and their families within the community to ensure high standards of physical health care are being carried out.
To ensure provision of holistic nursing assessment and high quality, safe, evidence-based care to inpatients.
Assessing, diagnosing, care planning, reviewing all aspects of physical care and supporting mental health nurses and HCAs in the community to also do this, upskilling staff when needed.
The post will support the wider Community Mental Health Rehab Team in making strategic improvements to the health and safety of patients through CQUINs and institute of health improvement projects. The post holders will also be encouraged and supported to make improvements to care that they identify on a day-to-day basis.
Help carry out audits to ensure that practise is safe and analyse data and formulate strategies to improve practice on the wards with the staff.
This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
