Living Well Support Service- Wellbeing Worker- Falls Prevention

Salary: FTE £24,661 per year
Hours: 2 x 24hr roles (example pattern 3 days 9am-5pm, 1 day 9-12 or 1-4).

If so, we are looking to recruit 2 X Wellbeing Worker’s to join our Living Well Support Service team Falls Prevention Winter Project, working across the Central Lancashire area.
The Living Well Support Service supports people to regain and maintain independence, helping to prevent hospital admissions and reduce the need for residential care. In the last year, the service supported over 9,000 people across Lancashire.
This winter, we are expanding the service with targeted support for older people at risk of falls, loss of independence, and avoidable hospital admissions, helping them stay safe and well at home during the winter months. The Falls Prevention Winter Project aims to keep older people out of hospital and reduce the frequent use of urgent and emergency care (UEC) services throughout winter, when vulnerable adults are at increased risk, by providing targeted interventions and support.

About the role:
As a Wellbeing Worker you will:
- Support older people who may be at increased risk of falls, loss of independence, or avoidable hospital admission.
- You will undertake person-centred holistic assessmentsto identify risks, unmet needs and develop personalised plans to improve safety, wellbeing and independence.
- Provide continued support for a designated period, to that build upon strengths and achieve personal goals.
- You will use a solution-focused approach to ensure that people can access the help that they need when they need it, either from Age UK Lancashire or from other community-based support.
To be successful in this role you will:
- Have a caring and empathetic attitude with a focus on helping people to maintain independence
- Be able to provide a person-centred, flexible and responsive service
- Have experience of managing your own workload, using decision making skills to manage and prioritise
- Identify environmental risks within the home and make appropriate referrals to project partners and other specialist services
- Be able to work well within a team and able to use your own initiative
- Have excellent communication skills with an ability to build relationships with individuals using the service, and other health care professionals and colleagues.
- Be able to use your digital skills to enable you to manage your time efficiently and effectively, including Microsoft applications such as Outlook and Excel and everyday use of iPad and smartphone devices.
- Able to manage a busy workload, keeping accurate records and conducting assessments
- Able to undertake general physical activity in people’s homes (often over 2 floors) for example providing light domestic duties
- A full driving Licence, with appropriate insurance cover, and be willing to transport patients in your own car.

This role is based at an AUKL Preston Base (PR2 area), you will make community visits across the Preston area, working in the community, hospitals and client’s homes. You will be required to attend meetings and spend time at our offices working with Age UK Lancashire colleagues.
