Care Coordinator – Full Time
Salary: £29,000–£31,000 per annum (dependent on qualifications and experience)
Aspects Care Ltd is continuing to grow, enabling us to support more individuals across our communities. Due to the expansion of our services, we are looking for an experienced, organised and motivated Care Coordinator to join our management team and help ensure the safe, effective and person-centred delivery of our services.
We are currently recruiting for a full-time Care Coordinator to support the Registered Care Manager and Deputy Care Manager with the day-to-day operational management of our supported living services. This is a 40-hour per week position worked over 7 days, including weekends and participation in the on-call rota.
At Aspects Care Ltd, we support adults with complex learning disabilities, behaviours of concern and mental health needs to live as independently as possible within their local communities through our supported living services. You will play a key role in ensuring that all support is delivered in accordance with person-centred values and that care plans accurately reflect each individual's assessed needs, preferences and outcomes.
As a Care Coordinator, you will be responsible for:
- Managing allocated service user staff teams.
- Conducting staff supervisions, competency observations and welfare meetings.
- Overseeing rota management to ensure safe and effective staffing levels.
- Carrying out quality assurance visits, project spot checks and visits to service user homes.
- Supporting recruitment, induction and ongoing development of staff.
- Developing and maintaining effective working relationships with service users, families, advocates and external professionals.
- Attending and actively participating in multidisciplinary meetings and staff meetings.
- Ensuring all documentation, records and compliance paperwork are completed accurately and in line with CQC regulations, company policies and best practice.
- Identifying opportunities to improve service quality and supporting the continuous development of the organisation.
This is primarily an office-based role; however, regular travel to our supported living services across the West Midlands is required, together with participation in the on-call rota.
Essential Requirements
- NVQ Level 3 in Health and Social Care (or equivalent) as a minimum.
- Previous experience in a Care Coordinator or similar supervisory role within health and social care.
- Experience supporting adults with complex learning disabilities, behaviours of concern and/or mental health needs.
- Excellent organisational, communication and leadership skills.
- Good IT skills and experience of maintaining accurate care records.
- A full UK driving licence and access to a vehicle insured for business use.
Successful applicants will be required to undergo an Enhanced DBS check. The cost of the DBS disclosure will be met by the applicant.
Salary: £29,000–£31,000 per annum, dependent upon qualifications and experience.
