To support vulnerable adults living in the community to manage their medication needs either with or without assistance to achieve their desired outcomes, including maximising their health, wellbeing, and independence, enhancing their quality of life, and taking a greater part in their local community.
The Medication Lead plays a crucial role in ensuring safe medication practices within the organisation. It is essential for this role to work in an enabling way, coaching, demonstrating, promoting, and encouraging independence. Through Medication management, take responsibility for the safe ordering, administration, and safekeeping of drugs and treatments. This involves ensuring that medications are administered as prescribed by a doctor or health professional, following organisational policies and guidelines,
An expectation of the role is that you will work flexibly across all areas with the Southend locality, work is carried out in service user’s homes and the local community.
Key Accountabilities:
- Responsible for promoting and facilitating the reporting of medication incidents, while encouraging a culture of learning from these incidents to prevent future occurrences.
- Ensure understanding and the proper assessment and mitigation of risk, including Information Governance, Safeguarding, Health and Safety and Business Continuity.
- Through observation and reporting, observe service users for any potential side effects related to medications. Ensure service users receive the correct dosage at the right time. If needed, provide reminders and assistance to service users in taking medications correctly.
- Weekly medication audits and flag tracking daily.
- Completing care plans and PRN protocols.
- Managing medication Incident reporting, by overseeing the process of reporting and learning from medication incidents. Working to improve reporting mechanisms and ensure that lessons are shared and acted upon.
- Ensure that service users get the medicines they need to effectively manage their condition in line with NICE guidelines SC1 & NG67.
- To ensure that Service Users have choice and control over the service/s they receive and the outcomes they wish to achieve.
- To show commitment to the mental and physical wellbeing of our service users by supporting them within a variety of settings, including: The Service Users home and their local community.
- Collaborate with other healthcare professionals to minimize harm from medications.
- Developing and maintaining good communication with customers, families, and carers, including where required - providing support to families and carers to help achieve and maintain the service users desired outcomes.
- Support with assessing staff competence in medication administration as and when required and in line with organisational KPI’s.
Key Skills:
- Previous experience in a similar environment supporting vulnerable people, is desirable but not essential.
- Can demonstrate an awareness of the needs of vulnerable people and the challenges they commonly face.
- Previous healthcare background in nursing, pharmacy, or social care would be beneficial. A caring and supportive individual who is passionate about providing a high-level care and support to vulnerable people.
- The ability to support people retain their dignity and respect their privacy when delivering medication support to them.
- Good listening and communication skills, both written and verbal are essential, with experience in building positive working relationships with colleagues and customers.
- Hold a full valid driving license.
