We are looking for an organised and compassionate Care Coordinator to join our growing team and play a key role in ensuring our clients receive the best possible support.
The Role
As a Care Coordinator, you will be responsible for managing and coordinating care services, supporting our care team, and ensuring our clients receive safe, reliable, and personalised care.
You will work closely with clients, families, care staff, and healthcare professionals to deliver an efficient and responsive care service.
Key Responsibilities
- Coordinate and schedule care visits, ensuring clients receive consistent support
- Manage rotas and allocate care staff effectively
- Build positive relationships with clients, families, and care workers
- Monitor and maintain high standards of care delivery
- Support recruitment, onboarding, and training of care staff
- Handle queries and provide ongoing support to the care team
- Maintain accurate records and ensure compliance with care regulations
- Assist with care plan reviews and service improvements
About You
We are looking for someone who is:
- Caring, organised, and highly reliable
- Confident communicating with people at all levels
- Able to work well under pressure and manage priorities
- Experienced in care coordination, domiciliary care, or a similar role (preferred)
- Knowledgeable of CQC standards and care regulations (desirable)
- Passionate about making a positive difference in people’s lives
