Contract to start ASAP until 06-12-26 with a possible extension.
Responsibilities:
- To regularly monitor, collate and update performance management information, reports and other monitoring information requirements to ensure all Welsh Government monitoring is accurate and compliant
- To have responsibility for supporting a caseload of participants using a holistic, enabling and collaborative approach
- To develop comprehensive individual action plans with clients and undertake further assessment and regular reviews as appropriate
- To ensure that all personal data is effectively protected and handled, within locally agreed Information Sharing Protocols
- To support individuals to address broad barriers to work, access to training, education, development opportunities and employment
- To ensure that all records and client files are kept up to date, including inputting into regular Triage meetings
- Fully adhere to the authorities Financial Procedures Rules and Procurement Procedures whilst ensuring all expenditure meets the eligibility criteria of the Welsh Government
- To keep in regular contact and track all participants following their completion from the project for up to three months
- To participate in all Welsh Government Communities For Work Plus Programme development activity including engagement activities
- Ensure equality, diversity, wellbeing, and sustainability are valued and embedded into services
- To adhere to all County Council policies to include GDPR, Protection of Vulnerable Adults and Children, Health and Safety Legislation, lone working guidelines
- To ensure that confidentiality is always respected and to ensure the safe keeping of IT equipment including laptops and other personal information held in files or electronically, whether when outreach working or on site
- To work on an outreach basis to engage with customers across Carmarthenshire
- To carry out other tasks and responsibilities of a similar nature as determined from time to time by the manager in relation to the smooth running of the service
- To provide flexible support to the programme across Carmarthenshire and work occasional unsocial hours on weekends and in the evenings, when required, to ensure effective delivery of the programme
