Job description:
We are looking for a dedicated Field Based Care Manager to oversee a designated area and ensure customers receive safe, person-centred, high-quality care. You will lead a team of Care and Support Workers, manage performance, and maintain strong compliance standards.
Key Duties

Complete and review person-centred care plans and risk assessments.

Carry out initial assessments, regular customer reviews, and respond to concerns or complaints.

Lead, supervise, and support Care and Support Workers, including training, spot checks, and performance management.

Manage rotas using our electronic rostering system, ensuring accurate call times and appropriate travel time.

Take part in the on-call rota, supporting emergency cover and operational needs.

Ensure customers are kept informed of any changes to their care.

Maintain accurate records and ensure all policies, procedures, and safety standards are followed.

Attend meetings, support service development, and contribute to continuous improvement.

Provide direct care when required and uphold excellent communication across the service.
This role requires strong leadership, excellent organisation, and a commitment to delivering outstanding care to our customers.
