Responsibilities
- Deliver personalized care according to each individual’s care plan, including assistance with daily activities such as bathing, dressing, and mobility.
- Support clients with medication management and health monitoring to promote their safety and health outcomes.
- Communicate effectively with clients, families, and multidisciplinary teams to ensure seamless delivery of care.
- Maintain accurate documentation of care provided, updates on client conditions, and any incidents or concerns.
- Assist with household tasks and ensure the environment remains clean, safe, and comfortable for clients.
- Use IT systems efficiently to record care activities and update client records in compliance with organizational policies.
- Drive or accompany clients using appropriate transportation methods when required for appointments or outings.
Skills
- Strong communication skills in English to effectively interact with clients, families, and team members.
- Ability to follow detailed care plans accurately while adapting to individual needs.
- Good IT literacy for maintaining electronic records and updating care documentation efficiently.
- Valid driving license and ability to drive safely when transporting clients or traveling between locations.
- Experience working within a care home or providing home care services is highly desirable.
- Knowledge of health and safety protocols relevant to healthcare settings.
- Excellent interpersonal skills with a caring attitude and the ability to build trust quickly.
