As a Dementia Support Co-Ordinator within the Bradford Dementia Hub, you’ll play a vital role in delivering a dynamic and innovative Dementia Information and Advice Service across the Bradford District. You’ll support individuals at every stage of their dementia journey — alongside their families and carers — providing personalised guidance, practical advice, and tailored support plans that empower people to live well and confidently.
In this rewarding role, you’ll manage a varied caseload, bringing a compassionate, person-centred approach to every interaction. You’ll organise community appointments and telephone consultations, co-produce meaningful personal plans and help individuals maintain independence, reduce isolation, and navigate challenges with dignity.

Who we’re looking for:

An understanding of barriers faced by individuals affected by dementia face, with the ability to identify practical ways to overcome them

Flexibility to adapt your approach to meet the needs to the clients and carers

Experience of working collaboratively across multi agency and partnership environments

Experience of working in a trauma informed and person-centred approach with individuals.
