We are excited to be launching a new outreach support service in the Pontesbury area and are looking for dedicated, caring, and motivated Support Workers to join our growing team.

About the Role As an Outreach Support Worker, you will provide person-centred support to individuals with learning disabilities, autism, mental health needs, and other complex support requirements.
No two days are the same, and you will play a vital role in helping people lead fulfilling and meaningful lives.
We run a multitude of Shropshire-based sites including Meadow Brook (Little Minsterley) The Granary, The Spinney, Heathland, Pine Crescent and Sunny Bank, ensuring safe, well-adapted homes for those we support.
Your responsibilities will include:
- Supporting individuals to access community activities and appointments
- Promoting independence and developing daily living skills
- Encouraging social inclusion and community engagement
- Providing emotional support and companionship
- Supporting with personal care where required
- Maintaining accurate records and support plans
- Working flexibly across a variety of shifts, including evenings and weekends

Essential Requirements
- Full UK Manual Driving Licence
- Willingness to drive service vehicles and support individuals in the community
- Right to work in the UK
- Excellent communication and interpersonal skills
Previous experience in care is desirable but not essential, as full training will be provided.
