Helix Medical Centre is based within Herts and Essex Community Hospital in Bishop's Stortford. We pride ourselves on delivering professional medical care in a friendly environment while striving to meet all the challenges of modern General Practice.
We are members of the local Primary Care Network providing extended services as a hub to the local community. We also work with Allied Health professionals together with an expanding complement of ARRs staff including Physicians Assistant, Clinical Pharmacist, Community Paramedic, Community Midwife, Health and Wellbeing Advisor, Care Co-ordinator, Social Prescribers and First contact physio, Dietician etc. There is a wide socio-economic and demographic served within our catchment area providing an interesting and varied case mix.
Job description
We are seeking a caring, organised and motivated Care Co‑ordinator to join our friendly GP practice team. This role combines personalised care and patient‑navigation responsibilities with essential front‑of‑house and administrative duties, supporting the smooth and effective operation of the practice.
You will play a key role in ensuring that patients—particularly those who are frail, elderly, or living with long‑term conditions—receive joined‑up, person‑centred support. As an integral member of our reception and administrative team, you will help patients access the right care at the right time while maintaining high standards of service and professionalism.
The postholder will work with a diverse range of people from a variety of cultural and social backgrounds. The ability to work confidently, sensitively and effectively in a varied and sometimes demanding environment is essential.
The successful candidate will have excellent communication and interpersonal skills, be patient, empathetic and highly organised, and will bring experience of working in health, social care or other support roles involving direct contact with individuals, families or carers.
Key Responsibilities
Personalised Care & Care Co‑ordination
- Proactively identify and work with people who would benefit from care co‑ordination, including those with complex needs.
- Support patients to develop and review their personalised care and support plans.
- Act as a central point of contact, helping patients navigate health, social care and community services.
- Liaise closely with GPs, nurses, social prescribers, health and wellbeing coaches and other MDT members.
- Assist with follow‑up after hospital care, transitions between services, and ongoing communication.
Reception & Front‑of‑House Duties
- Provide a welcoming, professional reception service to all patients and visitors.
- Book, amend and triage appointments (telephone, face‑to‑face and online).
- Process home visit requests, clinical messages and signposting queries.
- Handle incoming calls, emails, online requests and post.
- Support with test result queries and specimen handling in line with protocols.
Administration & Data Quality
- Register new and temporary patients and update patient information.
- Process repeat prescriptions following practice guidelines.
- Scan, index and code clinical documents using appropriate SNOMED codes.
- Maintain high‑quality record keeping and support data collection for audits/service improvement.
Professional Standards
- Maintain confidentiality and data protection at all times.
- Follow safeguarding, health & safety and infection control protocols.
- Promote equality, diversity and inclusive care.
About You
We are looking for someone who is:
- Empathetic, patient‑focused and a good listener.
- Organised, with strong time‑management and the ability to work under pressure.
- Confident communicating with patients, carers and professionals.
- Competent with IT systems (clinical system experience desirable; MS Office essential).
- Able to work well within a team, while also managing your own workload.
- Committed to personal development (training, including PCI‑accredited care‑coordination training, will be provided).
Essential: Previous experience in a public‑facing role and/or admin/reception environment.
Desirable: Experience supporting people with long‑term conditions or working in health/social care.

Hours of work: 38 hours per week as follows:
Monday 8am - 6.30pm
Wednesday 8am - 6.30pm
Thursday 8am - 6.30pm / 9am - 7.30pm (on alternate weeks)
Friday 8am - 6.30pm
Rate of Pay:
£12.71 per hour
Annual Leave: 5.6 weeks annual leave inclusive of bank holidays. Entitlements are pro-rata for part-time employees.
Pension:
Option to join NHS Pension scheme.
