An excellent and exciting opportunity has arisen within our Acute and Integrated Care Community Services at East Cheshire NHS Trust. We are looking to recruit an enthusiastic, compassionate and experienced Registered Nurse to join our Hospital at Home Team, providing support to the Virtual Ward and Urgent Community Response Teams, in collaboration with partners across health and social care within the East Cheshire region.
The role of the Hospital at Home Coordinator is to work with the multi- professional team contributing to the ongoing assessment, monitoring care and interventions in line with the service pathways and with advice from clinician(s). They will supervise other team members to ensure appropriate monitoring and support of the health and social care needs of patients on the Virtual Ward to prevent unnecessary hospital admission or early supported discharge. The Coordinator with be the liaison between a range of healthcare professionals, patients and their families to ensure patients receive the best care in their own homes.
Hours per week: 33.00

To act as a coordinator to facilitate and assist the day-to-day operations of the community urgent crisis response team and specialty virtual ward, providing clinical assessments and administrative duties such as coordinating care for patients, and acting as a liaison between patients, families, and health and social care professionals, taking on the management responsibilities necessary for successful service delivery.
To work collaboratively with Virtual Ward/Urgent Crisis Response Team Leader, Nurse Associate, and Patient Pathway Navigator to coordinate and facilitate the provision of accurate health and social care information, self-care, or health improvement advice or, where applicable, signpost to the appropriate service to meet the holistic and therapeutic needs of the patient.
