We are seeking a committed and experienced Care Coordinator to join our domiciliary care service based in Gravesend. This role is suited to someone who understands the realities of community care and can actively support service delivery, rota management, recruitment, and business growth.
Key Responsibilities
- Coordinate and manage care delivery within a domiciliary care setting
- Rota management, including:
- Supporting continuity of care
- Building and adjusting care runs
- Supporting new care packages and increased capacity
- Support carers with day-to-day operational queries and issues
- Participate in on-call support duties as part of a rota
- Liaise with clients, carers, and the office team
- Maintain accurate records on care management systems
- Support business growth, including:
- Assisting with onboarding of new clients
- Supporting the setup of new care runs and expansion of existing runs
- Support recruitment activities, including:
- Liaising with applicants and arranging interviews
- Supporting onboarding and induction processes
- Ensuring recruitment documentation is completed and recorded correctly
- Escalate concerns appropriately in line with company policies and procedures
- Client Management: managing clients, resolve concerns, assessment and care plan.
- Field Team Management: conduct supervisions, probations, competency assessment, and spot check
Essential Requirements
- Previous experience in a domiciliary care setting
- Experience in rota management, care coordination, or a senior care role
- Willingness and ability to participate in on-call support duties
- Strong communication, organisational, and problem-solving skills
- Reliable, committed, and professional approach to work
- Lives within 30 minutes’ driving distance of Gravesend
- Right to work in the UK
- Full UK driving licence and access to a vehicle
- Highly committed and flexible to meet service needs
- Support the on-call team outside office hours on an alternate-week basis
- Able to cover care shifts when required to ensure continuity of care
Desirable (but not essential)
- Experience supporting service growth or new care packages
- Experience in recruitment or onboarding within a care setting
- Experience using care management systems (e.g. Birdie)

Pay: £25,350.00-£27,300.00 per year
Experience:
- Care Rota Management: 1 year (preferred)
