We are looking for a Care Coordinator to join a great homecare branch in and around the Kent area
This position will see you liaise with staff, clients and external healthcare professionals to ensure the seamless delivery of organized and effective care whilst managing a team of care workers. Main responsibilities within this role include:
- Line manage and supervise Care Workers
- Organise and coordinate weekly, rotas and care services
- Update and evaluate Care plans and ongoing needs of clients
- Develop and maintain relationships with clients and care staff team
- Liaise with external healthcare professionals and organisations to ensuring the delivery of person-centered care
- Carry out on-call coordinating duties
- Assisting in recruiting and training
- Staff supervisions and spot checks and reviews as required
This position is office based Monday-Friday. Previous experience in the same role is required to be considered for this position.
