Role Purpose
The Community Physiotherapist will provide high-quality physiotherapy assessment, treatment, and rehabilitation services across a range of settings including:
- Inpatient Rehabilitation Units
- Care Homes and Nursing Homes
- Clients' Own Homes
- Supported Living Services
- Community Rehabilitation Services
The post holder will work collaboratively with Occupational Therapists, Rehabilitation Assistants, Nursing Teams, Consultants, Care Providers, Families, and Multidisciplinary Teams to deliver person-centred rehabilitation programmes that promote recovery, mobility, independence, safety, and wellbeing.
The successful candidate will manage a varied caseload including individuals recovering from illness, surgery, injury, neurological conditions, falls, frailty, and complex rehabilitation needs.
Key ResponsibilitiesClinical Responsibilities
The Physiotherapist will:
- Assess, diagnose, and treat clients presenting with a range of physical, neurological, musculoskeletal, and age-related conditions.
- Develop and implement evidence-based treatment plans.
- Deliver rehabilitation programmes focused on mobility, strength, balance, transfers, and functional independence.
- Complete comprehensive physiotherapy assessments.
- Monitor progress and adjust treatment plans appropriately.
- Prescribe and review therapeutic exercise programmes.
- Support service users to achieve their rehabilitation goals.
- Maintain accurate and timely clinical documentation.
Inpatient Rehabilitation Responsibilities
The Physiotherapist will:
- Assess patients admitted to inpatient rehabilitation units.
- Develop and deliver rehabilitation programmes aimed at facilitating recovery and discharge planning.
- Conduct mobility, transfer, balance, gait, strength, and functional assessments.
- Work closely with Occupational Therapists, Rehabilitation Assistants, Nurses, and Medical Teams.
- Participate in multidisciplinary team meetings and goal-setting reviews.
- Assist patients recovering from:
- Orthopaedic surgery
- Falls and fractures
- Medical illness
- Neurological conditions
- General deconditioning
- Contribute to discharge planning and transition back to home or residential settings.
- Support safe and timely discharge processes.
Community and Home Visit Responsibilities
The Physiotherapist will:
- Conduct physiotherapy assessments and treatments within clients' homes.
- Provide rehabilitation programmes aimed at maintaining or improving independence.
- Identify environmental risks affecting mobility and safety.
- Support clients following hospital discharge.
- Assess mobility and transfer needs.
- Recommend appropriate equipment and mobility aids.
- Liaise with family members, carers, and external healthcare professionals.
- Promote self-management and long-term wellbeing.
Care Home and Nursing Home Responsibilities
The Physiotherapist will:
- Conduct regular visits to care homes and nursing homes.
- Assess residents' mobility, balance, strength, and rehabilitation needs.
- Deliver individual and group rehabilitation programmes.
- Work with care staff to implement rehabilitation goals.
- Support residents following illness, injury, surgery, and hospital admission.
- Promote independence and reduce unnecessary dependency.
- Assist with falls prevention initiatives.
- Contribute to care planning and resident reviews.
Falls Prevention
The Physiotherapist will:
- Undertake comprehensive falls risk assessments.
- Identify intrinsic and extrinsic risk factors.
- Develop falls prevention strategies and action plans.
- Deliver strength and balance interventions.
- Recommend environmental modifications and equipment.
- Deliver falls prevention education to staff, residents, clients, and families.
Neurological and Long-Term Condition Management
The Physiotherapist will support individuals living with:
- Parkinson's Disease
- Stroke
- Dementia
- Multiple Sclerosis
- Frailty Syndrome
- Neurological Disorders
- Orthopaedic Conditions
- Long-Term Mobility Problems
The post holder will develop personalised treatment programmes to maximise function and quality of life.
Training and Education Responsibilities
The Physiotherapist will contribute to education and workforce development by delivering training sessions including:
- Falls Prevention
- Safe Mobility
- Strength and Balance Programmes
- Rehabilitation Principles
- Parkinson's Disease Awareness
- Moving and Handling
- Frailty Management
- Exercise Prescription
- Promoting Independence
The Physiotherapist may be required to provide training for:
- Care Home Staff
- Care Agency Staff
- Healthcare Assistants
- Rehabilitation Support Workers
- Families and Informal Carers
Professional Responsibilities
The Physiotherapist will:
- Maintain HCPC registration and CSP membership.
- Adhere to HCPC, CSP, and Therapies on Thames standards, policies, and procedures.
- Participate in clinical supervision and appraisal processes.
- Maintain accurate and legally compliant records.
- Produce high-quality clinical reports and recommendations.
- Participate in service development initiatives.
- Maintain Continuing Professional Development (CPD).
- Demonstrate professional accountability and autonomous practice.
Person SpecificationEssential Qualifications
- BSc (Hons) Physiotherapy or equivalent recognised qualification.
- Current HCPC Registration.
- Membership of the Chartered Society of Physiotherapy (CSP).
- Full UK Driving Licence and access to a vehicle.
Essential Experience
- Experience working as a Physiotherapist within rehabilitation, community, inpatient, elderly care, neurological, or musculoskeletal services.
- Experience conducting physiotherapy assessments and treatments.
- Experience managing rehabilitation caseloads.
- Experience working within multidisciplinary teams.
- Experience working autonomously.
- Experience delivering falls prevention interventions.
- Experience completing clinical documentation and reports.
