Overview
We are looking for a friendly, organised, and proactive Care Navigator to join our busy GP practice team. The successful candidate will play a key role in supporting patients, managing appointments, and ensuring patients are directed to the most appropriate healthcare professional or service. This role is for 25 hours a week, i.e. 5 hours a day. The hours are generally either 1pm to 6pm or 1.30pm to 6.30pm. Total weekly hours is 25 hours a week.
Patient Care & Care Navigation
- Direct patients to the most appropriate clinician or service based on their needs.
- Support patients with accessing healthcare services.
- Escalate patient concerns to clinical or management staff when required.
- Use EMIS Web and other practice systems effectively.
Appointments & Patient Administration
- Book, amend and cancel patient appointments.
- Process appointment requests via telephone, in person, and online systems.
- Manage home visit requests.
- Register new patients and update patient records.
- Process repeat prescription requests following practice procedures.
Front Desk:
- Welcome patients and visitors in a professional and friendly manner.
- Answer telephone enquiries courteously and efficiently.
- Manage patient queries and requests.
- Support all reception activities as required.
General Administration
- Scan and file documents into patient records.
- Support administrative tasks for the practice team.
- Send emails, photocopy documents, and complete routine office duties.
- Assist with patient recalls and follow-up communications.
Other Duties
- Support the smooth day-to-day running of the practice.
- Maintain confidentiality of patient information at all times.
- Follow practice policies, health and safety procedures, and safeguarding requirements.
- Undertake training and professional development as required.
About You
- Excellent communication and customer service skills.
- Confident using computer systems and learning new software.
- Organised with strong attention to detail.
- Able to work well under pressure in a busy environment.
- Professional, reliable, and adaptable.
- Previous GP practice, NHS experience is desirable but not essential.
Pay: £12.71 per hour
