Purpose of the Role
To support the management team in overseeing the care team, ensuring high-quality care is delivered to residents, while also providing leadership and supervision to the care team. As a Team Leader, you will be responsible for the administration, storage and ordering of medication, recording resident’s clinical observations and assisting the care team in all aspects personal care. Team Leaders are responsible for escalating the health needs of each resident to the appropriate healthcare professional in a timely manner. You will assist in the implementation of care plans and risk assessments, ensuring these are updated monthly in line with the residents’ individual needs. Team Leaders assist in completing quality audits to ensure regulatory compliance and sustainability.
The Care Team Leader responsibilities include but are not limited to the following:
- Ensure service users are at the heart of the care delivery and their wishes and preferences enhance their wellbeing
- Supervise staff and ensure that all staff contribute to the efficient running of the service
- Support service users to maintain their relationships and connections with the local community
- Monitor the wellbeing of each service user, being aware of any change impacting on care and care plans, ensuring that new needs are met, and written records are amended to reflect any variation, communicating updates to the Team Leader or healthcare professional.
- Be responsible for informing the management team and health professionals of any changes in the needs of service users
- Support service users with their medication requirements
- Act as a key point for contact with service users, their relatives and visitors, as well as health and social care professionals involved in their wellbeing, ensuring that any reasonable requests are acted upon, and concerns or complaints are escalated
- Be responsible for promoting and protecting the welfare of those individuals supported by the service
