Supporting Individuals Living with Dementia
Job Title: Health Care Manager
Working alongside: Two Registered Managers
Location: Wisteria House Woolwell
Job Purpose
· To provide effective leadership, clinical oversight and compassionate management within the care home, ensuring that residents living with dementia receive safe, person-centred, dignified and high-quality care.
· The Health Care Manager will work closely with the Registered Managers and the wider care team to ensure that residents' physical, emotional, psychological and social needs are understood and met.
· The role requires a strong understanding of dementia care, medications, safeguarding, risk management, care planning and regulatory requirements, together with the ability to lead and support staff effectively.
1. Person-Centred Dementia Care
· Ensure every resident receives care that is tailored to their individual needs, preferences, history and wishes.
· Promote dignity, choice, independence and respect at all times.
· Support staff to understand how dementia may affect communication, behaviour, memory and daily functioning.
· Promote positive approaches to behaviours that may be distressed or challenging.
· Ensure residents are supported to maintain relationships, interests, routines and meaningful activities.
· Encourage families and representatives to be actively involved in residents' care where appropriate.
2. Care Planning and Documentation
· Ensure care plans and risk assessments are accurate, current, person-centred and regularly reviewed.
· Monitor daily records, handovers, body maps, incident records and other relevant documentation.
· Ensure changes in residents' health or presentation are identified, documented and acted upon promptly.
· Support staff to understand and follow individual care plans.
· Carry out regular audits of care documentation and implement improvements where required.
3. Health and Clinical Oversight
· Monitor residents' general health and wellbeing.
· Ensure appropriate escalation when a resident's health deteriorates.
· Work collaboratively with GPs, district nurses, community teams, pharmacists, occupational therapists and other healthcare professionals.
· Ensure appropriate monitoring of nutrition, hydration, mobility, skin integrity, continence and pain.
· Promote effective falls prevention and pressure-area care.
· Support appropriate monitoring of long-term health conditions.
· Ensure healthcare appointments, referrals and follow-up actions are appropriately recorded.
4. Medication
· Promote safe medication management in accordance with legislation, organisational policies and residents' care plans.
· Ensure medication administration records are completed accurately.
· Monitor medication errors, omissions and incidents and ensure appropriate action is taken.
· Ensure staff administering medication are appropriately trained and competent.
· Pay particular attention to the appropriate use and monitoring of medicines for residents living with dementia.
5. Safeguarding
· Promote a culture where safeguarding is everyone's responsibility.
· Recognise and respond appropriately to safeguarding concerns.
· Ensure concerns, allegations and incidents are reported and escalated appropriately.
· Work with relevant safeguarding professionals and external agencies when required.
· Ensure residents are protected from abuse, neglect, discrimination and avoidable harm.
6. Leadership and Staff Management
· Work alongside the two Registered Managers to provide strong and consistent leadership.
· Lead, motivate and support care staff to provide high standards of care.
· Promote teamwork, professionalism and effective communication.
· Provide supervision, coaching and constructive feedback to staff.
· Identify training and development needs.
· Support new employees through induction and ongoing development within your role.
· Promote fair and consistent working practices.
7. Dementia Training and Best Practice
· Promote continuous learning and development in dementia care.
· Encourage staff to understand dementia and the impact it can have on residents and their families.
· Support staff to use appropriate communication techniques.
· Encourage meaningful activities and stimulation appropriate to each resident.
· Keep up to date with developments and recognised best practice in dementia care.
8. Quality Assurance and Compliance
· Support the Registered Managers in maintaining high standards of care and regulatory compliance.
· Participate in audits, inspections and quality improvement activities.
· Monitor incidents, accidents, and safeguarding concerns to identify trends.
· Develop and implement action plans where improvements are required.
· Ensure policies and procedures are followed.
· Promote a culture of openness, learning and continuous improvement.
· Contribute to preparation for regulatory inspections and reviews.
9. Infection Prevention and Control
· Promote effective infection prevention and control practices.
· Ensure staff follow appropriate infection-control procedures.
· Monitor cleanliness, PPE use, hand hygiene and outbreak procedures.
· Ensure infection-control concerns are identified and escalated appropriately.
10. Communication with Residents and Families
· Develop positive and professional relationships with residents and their families.
· Listen to concerns and respond sensitively and appropriately.
· Ensure families are kept informed about significant changes in residents' wellbeing, where appropriate.
· Support effective communication between care staff, management and external professionals.
· Promote an open and approachable management culture.
Essential Skills and Experience
· Experience working within a health or social care environment within a senior or management role
· Relevant Health and Social care qualifications.
· Experience supporting individuals living with dementia.
· Excellent communication and interpersonal skills.
· Strong leadership and organisational abilities.
· Good understanding of person-centred care.
· Knowledge of safeguarding and risk management.
· Understanding of care planning and quality assurance.
· Ability to identify and respond to changes in residents' health.
· Ability to lead, motivate and support a multidisciplinary care team.
· Ability to work calmly and professionally under pressure.
· Commitment to promoting dignity, independence and choice.
Desirable
· Experience with audits, inspections and quality improvement.
· Knowledge of relevant CQC requirements and adult social care legislation.

Pay: From £19.00 per hour
