We are seeking an enthusiastic and highly motivated Business Coordinator to join our Hospital Hub team. This is a key role supporting the coordination of hospital discharge pathways and ensuring individuals receive the right support at the right time to enable safe and timely discharge from hospital.
Working closely with health and social care professionals, you will manage referral processes, coordinate assessments and service activity, monitor performance data, and act as a central point of contact for partner organisations. You will play a vital role in supporting patient flow, reducing delays, and contributing to the delivery of high-quality integrated discharge services.
The successful candidate will have excellent organisational and communication skills, strong attention to detail, and experience of managing data and administrative processes within a fast-paced environment. You will be confident working collaboratively with a range of stakeholders and committed to delivering excellent customer service.
This is an exciting opportunity to make a real difference within a dynamic and evolving service, helping to ensure people experience a safe, effective, and person-centred journey from hospital to home.
If you are an organised problem-solver who thrives on coordination, partnership working, and improving outcomes for residents, we would love to hear from you.
