Overview
We're looking for a proactive and organised Care Coordinator to join our growing team. You'll play a key role in ensuring our clients receive high-quality, person-centred care while supporting our care staff and helping the branch operate efficiently.
Key Responsibilities
- Conduct client assessments, reviews, care plans and risk assessments to ensure care remains safe, effective and person-centred.
- Maintain regular communication with clients, families and healthcare professionals.
- Support medication reviews and respond appropriately to changes in client needs.
- Coordinate care schedules and help ensure all visits are covered efficiently.
- Participate in the on-call rota and provide care visits where required to maintain service continuity.
- Support the recruitment, induction, supervision and development of care staff, including Care Certificate completion and competency checks.
- Monitor quality standards through audits, spot checks, service reviews and incident investigations.
- Maintain accurate records, reports and compliance documentation.
- Process private client enquiries and payments while supporting the growth of the service.
- Build positive relationships with clients, staff and external professionals, contributing to continuous service improvement.
What We're Looking For
- Experience in a care coordination or senior care role within domiciliary care.
- Excellent organisational, communication and problem-solving skills.
- Good knowledge of care planning, risk assessments and CQC standards.
- Ability to manage competing priorities in a fast-paced environment.
- A flexible approach, including participation in the on-call rota.
- Full UK driving licence and access to a vehicle (preferred/required if applicable).
