Are you someone who enjoys helping people solve problems, build confidence and move forward in life?
We are looking for Community Mental Health Key Workers / Support Workers to join our team supporting adults with mental health needs, learning disabilities and/or autism to live more independent and fulfilling lives.
This role suits people who enjoy being active, getting out into the community and helping others overcome challenges to build confidence and independence.
This is not traditional care work. Our focus is on supporting people to build independence, develop life skills and become more confident within their communities.
One of the most rewarding parts of this role is seeing the progress individuals make over time as they gain confidence and independence.
About Us
New Forward Care was founded to support people with mental health issues, learning disabilities and autism to live their lives as independently as possible.
Many services and activities can be difficult for people to access without the right support. Our aim is to ensure individuals can access their community, take part in meaningful activities and receive the support they need to remain happy and fulfilled.
Our office is based in Maltby, Rotherham, however the majority of the role involves supporting individuals across the wider Rotherham community.
The Role
As a Key Worker you will support individuals primarily on a 1:1 basis within the community (occasionally supporting small groups of up to two people).
You will help individuals build confidence, develop independence and work towards personal goals.
Responsibilities may include:
- Supporting individuals 1:1 in the community
- Encouraging people to build independence and daily living skills
- Supporting individuals to access community activities and opportunities
- Supporting with shopping, appointments and budgeting
- Helping individuals build social connections
- Planning and organising individual activity plans (with support from management)
- Supporting and occasionally facilitating small group activities
- Recording activities and tracking progress towards independence goals
- Reviewing progress and taking feedback (with support from management)
- Attending training and working in line with company policies and procedures
- Reporting incidents, concerns or complaints where required
- Helping maintain a safe and positive environment within the service
Community-Based Role (Driving Required)
This role involves supporting individuals across the Rotherham area within the community.
Because of this:
- A full UK driving licence is required
- Access to a vehicle for work use is required
- You will support individuals both in their homes and out in the community
Mileage reimbursement is provided where applicable.
