The west adult learning disability community team has an opportunity for a full time band 5 occupational therapist to join.
The job involves being a member of a multi-disciplinary community learning disability team providing Occupational Therapy assessment, advice and interventions for adults with learning disabilities
who have complex needs.
You will have or develop the knowledge to use Model of Human Occupation and associated assessments to identify person centred needs and guide your Occupational Therapy practice, including assessment, planning, implementing and reviewing .
You will develop knowledge and skills of effective approaches for adults with a learning disability under the guidance of senior staff.
You will liaise with clients, families, members of the MDT and other agencies.
You will complete reports in an accessible format to ensure Occupational Therapy recommendations are understood and implemented.
You will provide training to clients and carers about how to facilitate purposeful engagement in selfcare, productivity and leisure activities .
You will facilitate safe and effective discharge from the Learning Disability team.
You will participate in the planning, development and evaluation of Occupational therapy services for adults with a learning disability
