Fixed-Term Maternity Cover
Job summary
Wirral Multicultural Organisation is looking to appoint an energetic, compassionate and community-focused HealthLink Community Link Worker to join the organisation on a fixed-term maternity cover contract.
The successful candidate will engage with local ethnic minority communities, particularly people who may be isolated, older, living with long-term health conditions, or experiencing language and cultural barriers when accessing healthcare.
The role supports people to access healthcare and understand how to navigate local health services. You will provide practical assistance, advocacy, signposting, health-promotion and prevention advice, helping individuals access primary care services, screening appointments and other appropriate health and wellbeing support.
You will assist clients with matters including GP registration, healthcare appointments, NHS correspondence, prescriptions, referrals and access to dentists, opticians and other relevant services. The service aims to encourage early detection, improve access to treatment and support the effective management of identified health conditions.
Interpreting and translation are also delivered as part of this role. The postholder will be required to travel between WMO, GP practices, hospitals, healthcare settings, community locations and interpreting assignments across Wirral when needed.
Main duties and responsibilities
- Support clients to register with GP practices and access primary healthcare services.
- Assist clients with arranging, understanding and attending healthcare appointments.
- Help clients understand NHS letters, appointment information, prescriptions and healthcare correspondence.
- Provide advocacy and practical support when clients experience difficulties accessing services.
- Liaise with GP practices, hospitals, healthcare professionals and community organisations.
- Signpost and refer clients to appropriate health, wellbeing and community support services.
- Support clients to access dentists, opticians and other relevant healthcare providers.
- Help clients understand referral processes and follow-up arrangements.
- Provide health-promotion and prevention advice appropriate to clients’ needs.
- Encourage attendance at primary care services and screening appointments.
- Provide interpreting and translation support when required.
- Travel to interpreting assignments, healthcare settings and community locations across Wirral when needed.
- Maintain accurate, confidential and up-to-date client records.
- Work in accordance with safeguarding, GDPR and WMO policies.
- Support community outreach, health-promotion activities and engagement events.
- Promote the HealthLink service within the community and among partner organisations.
- Contribute to monitoring, reporting, case studies and achievement of project outcomes.
- Attend team meetings, training and partnership meetings as required.
- Carry out other reasonable duties connected with the role.
Essential requirements
- Fluent in spoken and written Bengali.
- Good spoken and written English.
- Experience in community support, healthcare, advocacy, interpreting, social care or another relevant client-facing role.
- Excellent communication and interpersonal skills.
- Ability to communicate sensitively with people from different cultural backgrounds.
- Ability to work independently and as part of a wider team.
- Good organisational and time-management skills.
- Ability to maintain accurate records and manage confidential information.
- Basic IT and administrative skills.
- Understanding of equality, diversity, safeguarding and confidentiality.
- Ability to travel reliably between WMO, healthcare settings, community locations and interpreting assignments across Wirral.
- Legal right to work in the United Kingdom.
Desirable requirements
- Ability to speak Urdu, Hindi or another community language.
- Previous experience of working with NHS services.
- Experience of interpreting or translation.
- Experience of supporting people facing language or cultural barriers.
- Knowledge of local healthcare and community services across Wirral.
Working arrangements
- Hours: 15 hours per week
- Working days: Normally Monday and Friday
- Location: WMO, GP practices, hospitals, healthcare settings and community locations across Wirral
- Contract: Fixed-term maternity cover
- Pay: £12.90 per hour
