Job Purpose
As a Home & Community Services Field Care Supervisor you will be responsible for looking after a group of Service Users and Care Workers within the local community. This will include carrying out risk assessments, monitoring and reviewing care plans, supervising Care Workers and ensuring all Service Users care visits are allocated and running to the high standards expected by APASEN. Responsible for the efficient running of an area and management/supervision of Care/Support Workers and care provided to Service Users within that area.The aim of the position is to enable service users to remain safely in their own home with as much independence as possible. Alongside ensuring, through regular communication, review and continuous monitoring, that care and support is provided at all times in a professional, discreet and caring manner, always ensure all service users maintain the right to privacy and dignity.
Involvement and Information
❖ Implementation of quality control procedures and the quality assurance process
❖ To carry out Service User needs assessments, care/support plans and risk Assessments
❖ Attending Service User’s first call to introduce the Care/Support Worker to the Service User
❖ To organise and attend Service User reviews, including reviewing and updating of needs assessment, care/support plan and risk assessment documents
❖ To liaise closely with Care Co-ordinators to ensure correct Service User and Care/Support Worker matching for new Service Users or in instances of holiday/sickness cover
❖ To assist Care Co-ordinators allocate unassigned calls during office hours as appropriate/required
❖ To allocate all unassigned calls during on-call hours – where visits can’t be assigned, undertake the visit
❖ To ensure that all changes to care schedules are reported, without delay, to the Care Co-ordinator.
❖ Develop teamwork and efficient working practices (including Health and Safety) and ensure that Care Assistants have the required knowledge and equipment to carry out their jobs efficiently and safely.
❖ To ensure the necessary Risk Assessments are done and staff are following the correct procedures
❖ Undertake on-call duties as directed and record/report all on-call activity to the office
❖ Undertake field based training to ensure Care/Support Workers are working within their competency level
❖ To ensure all reporting and recording documents are kept up-to-date
❖ To liaise closely with colleagues and other care professionals
❖ To identify, train and develop a suitable deputy as directed
❖ Monitor the service to individuals against agreed quality standards on a regular basis and report to the management team to ensure ongoing provision of a high quality service.
❖ Ensure the safe administration of medication to Service Users (including direct administration) is carried out in accordance with Directorate policy and procedures in order to ensure safety of Service Users
❖ Maintain and update systems and records to ensure clerical and management information is available for decision making.
❖ Ensure that changes in Service Users needs or Health and Safety problems are communicated to the Team Manager so that care is safely delivered.
❖ Take responsibility for the day to day problems and issues raised by Service Users and inform the appropriate officer to ensure the matter is resolved.
❖ To ensure the health and well being of Service Users by participating in the planning, coordination and provision of appropriate group and individual activities.
❖ Liaise and communicate with other agencies, carers, service provides etc to ensure consistency of individuals care plans to be actioned and addressed as a team.
❖ Undertake such other work as may be determined by the Manager from time to time, up to or at a level consistent with the principal duties and responsibilities of the job.
❖ To assess Care/Support Workers against Skills for Care Common Induction Standards during shadow working sessions
Personalised Care, Treatment and Support
❖ To ensure that the delivery of the care and support by the care worker team reflects the services user’s needs, desired outcomes, preferences and diversity, promoting their wellbeing and quality of life
❖ To ensure that the care worker team assist service users with mobility problems and other physical disabilities including incontinence, using aids and personal equipment i.e. hoists, assistive technology etc. in line with their individual care plans
❖ To communicate openly, honestly and effectively with service users, carers and other professionals involved in their care and support plan.
❖ To report any concerns to your line manager relating to significant changes in the service user’s health and support needs.
❖ To undertake as required, individual service user care and support planning and formal reviews, with the service user and all other involved parties. Use your knowledge and expertise of person centred care to ensure that the service user is able to meet their desired outcomes, in a safe though non risk adverse manner, promoting their independence at all times.
❖ To be part of the “on-call” rota to ensure that emergency support is provided as and when required – emergency support is provided on a 24 hour basis.
Safeguarding and Safety
❖ To understand the arrangements for ensuring that service users are safeguarded against the risk of abuse.
❖ To ensure that the care worker team can identify situations with the potential for abuse, take preventive measures, respond to allegations of abuse and report these in accordance with Government, corporate and local policies.
❖ To provide a confidential way for staff to raise concerns about risks to people who use the service, poor practice and adverse events in line with the Organisation’s whistle-blowing policy and legislative requirements.
❖ To, in accordance with Oganisation and local policies:
- identify the potential for abuse and take appropriate preventative measures
- respond to allegations of abuse
- represent the Oganisation by attending any external and internal safeguarding investigation meetings,as required by your line manager
❖ To report any untoward incidents or accidents to your line manager and handle these in accordance with Organisation policy and quality monitoring processes. To regularly assess the general standards of hygiene and cleanliness in accordance with planned care and support, identifying any risks of infection or contamination and implementing remedial action plans.
Pay: £25,585.00 per year
