The Home Care Hub is looking for compassionate, reliable and experienced Support Workers to provide high-quality, person-centred support to older people in their own homes and within the community.
Our support is tailored to each person’s individual needs, preferences and routines, helping them maintain their independence, dignity and wellbeing while enjoying a safe, fulfilling and meaningful quality of life.
The Role
As a Self-Employed Support Worker, you will provide practical, emotional and social support to older people who may require assistance with everyday tasks, companionship, mobility, maintaining their independence and accessing their local community.
You will take a person-centred approach, promoting dignity, choice, independence and inclusion while supporting individuals to remain in their own homes and continue to live life on their own terms.
Your responsibilities may include:
- Providing friendly, reliable companionship and emotional support.
- Supporting older people in a patient, respectful and compassionate manner.
- Assisting with everyday household tasks to help maintain a safe, clean and comfortable home environment.
- Supporting with shopping, including accompanying individuals to shops or completing shopping on their behalf where appropriate.
- Supporting with meal preparation, taking individual preferences, dietary requirements and independence into consideration.
- Providing support with mobility and everyday activities, where appropriate and within your agreed responsibilities.
- Encouraging and supporting community engagement, including attending appointments, social activities, clubs, religious or community groups, family visits and other activities.
- Providing medication support in line with the individual's care plan and agreed responsibilities.
- Supporting individuals to maintain routines and continue participating in activities that are important to them.
- Encouraging independence and helping individuals retain confidence in managing everyday tasks.
- Providing reassurance and companionship to individuals who may experience loneliness or social isolation.
- Communicating effectively with individuals, families and relevant professionals where required.
- Maintaining appropriate professional boundaries and respecting confidentiality at all times.
- Recognising and reporting any concerns relating to an individual's wellbeing, safety or safeguarding.
- Recording and reporting relevant information in accordance with The Home Care Hub's procedures.

Essential Requirements
- Minimum 1 year of relevant Support Worker/Care experience.
- Self-employed status or willingness to work on a self-employed basis.
- A DBS check registered on the Update Service.
- Have your own private carer's insurance that covers you for providing support/care services.
- Have access to a reliable vehicle and be able to travel to clients where required.
- Have your own car insurance with a policy that includes business miles.
- Experience supporting older people is highly desirable, particularly experience with companionship, mobility support, dementia or age-related needs.
- The ability to provide suitable references.
- A commitment to safeguarding, confidentiality, dignity and person-centred care.
