Case Worker Temp Contract Fulltime
£13:50hr plus holiday pay

BRC are currently recruiting for a Case Worker to join a dedicated and supportive team delivering community based dementia support across Cardiff. This is a fixed term opportunity offering a rewarding and varied role where no two days are the same.
This is a fantastic opportunity to support individuals living with dementia to remain active, achieve personal goals, maintain independence, and improve their overall wellbeing.
Key responsibilities include:
- Providing one to one support to individuals living with dementia
- Completing assessments and developing person centred support plans
- Supporting individuals to achieve meaningful goals and activities
- Facilitating group activities within community and care settings
- Providing practical and emotional support to families and carers
- Working alongside volunteers, health professionals and partner agencies
- Maintaining accurate records and case management systems
Experience:
- Experience within health and social care, support work or community services
- Experience supporting individuals with dementia is desirable
- Ability to work independently and manage a varied caseload
- Strong communication and relationship building skills
- Full UK driving licence and access to own vehicle
- Good IT and administration skills
This role would suit someone who is compassionate, proactive and passionate about supporting people to live well with dementia.
