Our Community Homeless Health Team supports individuals who are homeless, sofa surfing and in temporary and emergency accommodation. We help 'bridge the gap' and promote re-engagement with health care.
As a Community Homeless Health Support Worker, you will work alongside registered healthcare professionals to deliver compassionate, person-centred care and support. This is a rewarding opportunity to make a real difference in the lives of individuals who often face significant barriers to accessing healthcare.
You will proactively engage with people experiencing health inequalities across a variety of community settings, building trusted relationships and empowering them to make positive changes to their health and lifestyle. Through your support, you will help individuals access services, improve their wellbeing, and achieve better health outcomes.
In this rewarding role, you will play a key part in tackling health inequalities by working with some of the most vulnerable members of our communities. Providing support to people across Bristol, North Somerset and South Gloucestershire (BNSSG) who are homeless, vulnerably housed, or recently discharged from hospital.
This is a role that brings new challenges, opportunities, and the chance to make a lasting impact on someone's journey towards better health and stability.
To carry outreach plans as prescribed by registered therapists, in accordance with instructions and recognised training received. This will involve a level of autonomy and decision making using protocols as drawn up by the service.
Provide assistance to clinical staff in the assessment and treatment of patients on discharge from hospital
Follow a prescribed care plan to support service users at home including support with access to GP surgeries, outpatient appointments, general health care
To participate and support in promoting healthy and independent living practices to patients
To work collaboratively with other organisations across different sectors of health and social care – for example St Mungo’s
To carry out ge
neral administrative and non-clinical work to contribute to the functioning of the team such as photocopying, filing, liaison and telephone calls.
