At The Care & Support Group, we’re excited to be expanding our services in Sunderland following the award of a new contract. Our Houghton-le-Spring office will act as the base for this growing service, creating opportunities for experienced care professionals to play a key role in shaping high-quality community care from the very beginning.

The Role
We are seeking an experienced and compassionate Field Supervisor to support our Sunderland services. This is a varied, hands-on role combining client assessments, care planning, staff support, and direct care delivery when required.
You will be the first point of contact for new clients, working closely with them and their families to understand needs, develop robust care plans, and ensure care is delivered in a way that achieves positive outcomes and promotes independence.
Key Responsibilities
· Complete initial and ongoing assessments with new and existing clients
· Develop, review, and maintain high-quality, person-centred care plans
· Work with clients to ensure care needs are met and desired outcomes are achieved
· Support care staff in the community, assessing practice and ensuring duties are delivered safely and effectively
· Carry out spot checks, observations, and quality monitoring
· Provide guidance, mentoring, and practical support to care teams
· Deliver hands-on care when required to support service continuity
· Liaise with Care Coordinators, management, families, and external professionals
· Ensure compliance with CQC standards, safeguarding, and company policies
What We’re Looking For
· Strong background in domiciliary care or community-based services
· Proven experience completing assessments and writing detailed care plans
· Excellent understanding of person-centred care and outcome-focused support
· Confident supporting and mentoring staff in the community
· Flexible, proactive, and able to respond to changing priorities
· Willingness to provide hands-on care when needed
· Excellent communication and documentation skills
· A full UK driving licence and access to a vehicle (essential)
· Eligible to work in the UK
· Level 3 health and social care (or working towards)
