We have an exciting role on offer within our Access and Safeguarding service as a Social Care Worker.

This is an exciting opportunity for someone with extensive experience and qualifications in Adult Social Care looking for a new challenge.
The role involves working directly with using the conversational, person-centred and trauma informed approach promoting independence and discussing how to support individuals to meet their goals and aspirations.
You will undertake strength-based conversations under the Care Act 2014 and review the current support plans to establish whether they continue to meet the desired outcomes of both service users and carers alike.
You will undertake planned reviews of services within the community services (home care, day care), S117 after care, residential and nursing placements, support using direct payments, and carers support.
You will work with experienced and settled teams, holding an interesting and varied caseload.
You will have the opportunity to take a blended contract, working from home as well as working from Fountain Court, our open planned and modern office. You will have the opportunity to work collaboratively with other Adult Social Care teams.
