Access Your Care is an established leading home care provider in North Somerset. Due to growth in our business we are looking to recruit a Care Coordinator to join our team.
A full driving license and access to your own vehicle is required for this role, as is experience in the Health & Social Care sector. You responsibilities will include:
- Initial client assessments, helping to build the clients care plan.
- Follow up's with clients when there is a change in circumstances.
- Writing and updating client bios, ensuring all information is correct and up to date for our community teams.
- Being the link between clients, families, care teams and professionals to ensure high quality care is always delivered.
- Resolving client and community team enquiries.
- Acting as the on call manager as part of the manager on call rota.
- Delivering care to clients where required.

Financials
- This is a salaried role, starting at £27,050.00 for your base pay.
- Earn up to £4,872.20 as part of the manager on call rota.
- Get the proud to care bonus, paid to you every six months for working for a North Somerset strategic provider. Worth up to £1,227.00 per year.
- Add that all together – £27,050.00+£4,872.20+£1,227.00=up to £33,149.20 earnings per year if working full time.
- Receive a £1,000 joining bonus for coming on board with us.

- You will be part of the 8 week on call rota, working evenings and 1 weekend in 8
- Up to 33 days annual leave per year depending on length of service.
