We are thrilled to announce an excellent opportunity for an experienced Social Prescriber to undertake holistic assessments and co-design Health and Well-being Plans with individual patients, identifying support needs to ensure maximum engagement in improving health and well-being, to join our team at West Moors Village Surgery under the Additional Role Reimbursement Scheme (ARRS).
The Social Prescriber supports individuals to improve their health, wellbeing, and social connectedness by helping them access community services, activities, and support.
Working closely with GP practices, healthcare professionals, voluntary organisations, and community groups, the Social Prescriber empowers people to take greater control of their health and address non-medical factors affecting their wellbeing.
Experience working in a Primary Care setting preferred, however we provide the right candidate with the necessary training to develop a rewarding career within primary care.
We are seeking an independent, hardworking professional who thrives on being valued for their contributions. The ideal candidate will possess a calm and confident approach, excellent communication skills, and the ability to perform relevant duties at a high level.
