We are looking to appoint a Transitions Social Worker to join our newly developed Preparation for Adulthood (PfA) Service within SEND Central in York.
This role sits within Adults Social Care and works at the interface between children’s and adult services, supporting young people with SEND to move into adulthood in a timely, planned and person-centred way, in line with York’s Preparation for Adulthood vision.

You will:
-
Lead on Care Act assessments and support planning
- Manage a complex caseload
- Work collaboratively across agencies to support effective, outcome-focused transitions
- Contribute to early planning (from Year 9 onwards)
- Promote strengths-based practice, supporting independence, employment, community inclusion and positive adult lives
About you:
-
Qualified Social Worker, registered with Social Work England
- Experience of complex practice and multi-agency working
- Strong knowledge of the Care Act and Mental Capacity Act
- Confident in decision-making and managing complexity
- Committed to supporting young people to achieve independence and positive outcomes
