We are seeking a proactive and compassionate Health & Wellbeing Coach (Level 5 equivalent) to join our Primary Care Network team under the Additional Roles Reimbursement Scheme (ARRS).
This role is central to delivering the PCN DES personalised care agenda, supporting patients to take greater control of their health through structured coaching and behaviour change interventions. The post holder will focus on improving outcomes across key public health priorities, including smoking cessation, cancer screening uptake, low-level mental health and wellbeing, and weight management.
Working as part of a multidisciplinary team, the Health & Wellbeing Coach will provide holistic, person-centred support, particularly for patients with long-term conditions or those at risk of developing them, helping to reduce health inequalities and demand on primary care services. The post holder will be based within 1-2 practices with our PCN.
Key Duties and Responsibilities
- Deliver personalised health coaching interventions in line with PCN DES requirements, supporting patients to improve self-management and overall wellbeing.
- Manage a defined caseload, providing structured 1:1 coaching sessions using evidence-based behaviour change techniques (e.g. motivational interviewing).
- Provide targeted interventions to support:
- Smoking cessation, including assessment, Coaching and options regarding supportive smoking aids.
- Cancer screening engagement, proactively encouraging and supporting uptake of national screening programmes (bowel, breast, cervical)
- Low-level mental health and wellbeing, including stress management, building resilience, and social wellbeing support
- Weight management, including healthy lifestyle coaching, nutrition education, and physical activity promotion supporting the Tier 2 weight management programme.
- Support individuals to set personalised goals and develop shared care plans, aligned with personalised care principles.
- Work alongside Social Prescribing Link Workers, GPs, nurses, pharmacists, and wider MDT colleagues to coordinate care and ensure joined-up support.
- Identify and address health inequalities and barriers to engagement, proactively reaching underserved groups. Run regular searches to identify Cohorts from patient data.
- Deliver group-based interventions, workshops, and health education sessions where appropriate.
- Accurately record patient interactions, outcomes, and activity data on clinical systems, ensuring compliance with GDPR and PCN reporting requirements.
- Contribute to achieving PCN targets and outcomes, including prevention, personalised care delivery, and population health management.
- Signpost and refer patients to appropriate community, voluntary sector, and secondary care services.
- Maintain up-to-date knowledge of NHS Long Term Plan priorities, PCN DES specifications, and public health initiatives.
- Data Collection be able to collate workload data accurately, timely each month,
- The above reflects most of the work carried out by the Health and Wellbeing coach, but may change according to the commitments required to support the PCN and NHS DES.
- Training Attend Regular PLT Training and maintain all Mandatory Training as required.
Skills and Experience
- Level 3 qualification (or working towards) in health coaching, behaviour change, or relevant health/social care field (as aligned with PCN DES expectations).
- Experience in a health, wellbeing, or community-based role, ideally within primary care, public health, or voluntary sector settings.
- Demonstrable understanding of personalised care approaches and behaviour change methodologies.
- Knowledge or experience in at least one of the following:
- Smoking cessation interventions
- Cancer screening awareness and engagement
- Mental health and wellbeing support
- Weight management and lifestyle interventions
- Strong communication and engagement skills, with the ability to build rapport with diverse patient populations.
- Ability to deliver both 1:1 and group-based interventions effectively.
- Competent in managing a caseload and working to targets and outcome measures.
- IT literate with experience using clinical systems and digital platforms.
- Understanding of safeguarding, confidentiality, and data protection.
