We are seeking a compassionate and resilient Mental Health & Addiction Triage Practitioner to join our Customer Service Centre team. In this important role, you will review and triage clinical referrals for Priory's addiction services, helping individuals access the most appropriate support for their needs.
This role requires flexibility to work a shift pattern covering 8:00am to 8:00pm Monday to Friday, alongside a Saturday morning service. We welcome applications from individuals with professional or lived experience of addiction and mental health, who can demonstrate empathy and a commitment to supporting people at often challenging times in their lives.
This role can be completed as office based, hybrid or remotely however we require the post-holder to live within a reasonable commute to our Customer Service Centre in Stockton-On-Tees. Hours will be between 8am and 8pm, 5 days per week. Some weekend work may be required.

As a Mental Health & Addictions Triage Practitioner, you will review and triage referrals, assessing individuals' needs and directing them to the most appropriate treatment pathway. You will provide guidance and signposting where alternative support is more suitable, while managing patient enquiries with empathy and professionalism.
You will assess and escalate risk and safeguarding concerns, maintain accurate clinical records, and work closely with referral coordinators and clinical teams to ensure a seamless patient journey. Acting as a clinical point of contact within the Customer Service Centre, you will support colleagues, contribute to safe and effective case management, and participate in supervision and ongoing professional development.

You'll have a relevant qualification or be working towards one, such as a Level 3 Health & Social Care qualification, Psychological Wellbeing Practitioner training, trainee therapist status, or equivalent experience.
You'll have an understanding of mental health and addiction, with the ability to assess referrals, identify and manage risk, and make informed decisions about appropriate care pathways. Experience in adult mental health, addiction services, primary care or a similar setting is desirable.
You'll be an empathetic and confident communicator, able to build rapport quickly, handle sensitive conversations professionally, and work collaboratively with colleagues and patients. Strong organisational skills, sound judgement and a commitment to high standards of care are essential.
