This is an exciting opportunity to join a fast-paced and growing organisation.
We’re looking for Nurse – Community Outreach for our Kingston and Merton Services.
Working with a Target Priority Group of Rough Sleeping Service users, you will work autonomously across multiple boroughs and community settings, carrying out complex assessments, decision-making, care planning and risk management for patients with highly complex needs.
The post holder requires advanced clinical expertise, independent judgement, leadership, service development, and specialist support to multidisciplinary teams. Act as a recognised source of specialist expertise, supporting and influencing clinical practice across teams.

Salary: £43,606 - £51,867 per annum including London Weighting where applicable
